Passport to Health

We're excited to announce the launch of our new "Passport to Health" program. Every month, we'll be giving you a fun, simple challenge to improve your overall health and well-being. If you complete it, you get a stamp on your "patient passport" and a free prize! Once you fill your passport with stamps, you'll get an even bigger bonus prize. Call or stop by the office to get your passport and get started! And be sure to join our Passport to Health Facebook group for tips, tools and support on your health journey.


February is American Heart Month. Heart disease is the leading cause of death for both men and women of all ages, so February's challenge is to do at least one of the following to boost your heart health:

  • Get a blood pressure check. We do blood pressure checks at the start of every visit here at the office, so schedule an appointment if you're due for an exam or follow up. Or join us on February 22nd for a FREE blood pressure check at our Heart-to-Heart event.

  • Get your blood pumping by trying a new workout. Is there an online yoga class you've been wanting to try? A new hiking trail you've been dying to explore? A new song you've been wanting to dance around the house to? Take the opportunity this month to move your body in some fun way that calls to you.

  • Quit smoking. This is the single best thing you can do for your heart if you're a smoker. If you need support, please come in to talk to one of our providers. Our on-staff social worker can also provide you with plenty of resources to help you break the habit. You can also read these top tips for quitting from the American Lung Association.

Once you've completed at least one of the above during the month of February, call or stop by the office so we can stamp your passport and get you your reward!
